This is a daunting task, especially for someone under extreme stress - it is just hard to ponder and formulate all of the necessary information. This is a beautifully created resource for creating that plan. I especially love that it not only has the usual spaces for contact numbers and other such information, but it also has beautifully designed pages to fill in and help ground and note simple skills needed to help get through those especially stressful moments. Thank you for this resource!
